Organization Request for New Web site or E-mail Address

This form is for requesting organizational Web sites or e-mail address. The form allows us to determine who is ultimately in charge of the site or e-mail account and who is doing the actual work (Technician). Use only BSC e-mail account in the form below. Authentication by our main web server is accomplished via this account.

Definitions:

Student in Charge- Person responsible for performing actions which create web pages and place them on the web server.
