Internship Agreement Form

Forms should be returned to Katy Smith or to your course instructor prior to your first day of work.

Off-campus Projects/Contracts Release Form

Students completing internships locally, nationally, or internationally should sign this form. Please return signed form to Katy Smith before beginning your internship.

Internship Evaluations

Evaluations will be emailed to both the student and the employer at the end of the internship. Results will be shared with supervising faculty.
