Release Forms

Directions:

Anytime you travel off campus for the purpose of representing the College or earning academic credit, the College must have a signed release form on file in order for you to participate. Please read the descriptions below to determine which release form is appropriate for your experience. You must be at least 19 years old (legal contractual age in the state of Alabama) to sign without the signature of your parent/guardian. Submit the signed form to your faculty sponsor if you are participating in a Birmingham-Southern sponsored project or to the Office of Exploration Term and Contract Learning if you are contracting an individualized study.

Travel Abroad Release
Use this form when you are traveling outside of the United States with a Birmingham-Southern sponsored project. Example: During the Exploration Term, you participate in the study/travel project "The Classical and Renaissance Worlds of Italy."

Domestic Trip Release
Use this form when you are traveling outside of Alabama and within the United States with a Birmingham-Southern sponsored project. Examples: During the Exploration Term, you participate in the project "Industries of the Southern Coastal States," or during the Spring Term, you and some of your fellow students represent the College at a conference in Philadelphia.

Independent International Travel Release
Use this form when you are traveling independently outside of the United States to represent the College or to earn academic credit. Example: During the Summer Term, you contract an individualized senior project to study biology in Costa Rica. This travel release is appropriate for international study approved through the Exploration Term Office or the Office of International Programs

Independent Domestic Travel Release
Use this form when you are traveling independently outside of Alabama and within the United States to represent the College or to earn academic credit. Example: During the Exploration Term, you contract an internship with NBC in New York City.

Projects/Contracts Release
Use this form when you are traveling within Alabama either independently or with a group from BSC to represent the College or to earn academic credit. Examples: During the Fall Term, you participate in a field trip to Montgomery as part of your class, or you contract an individualized project to intern in a neuropsychology lab at UAB.

Travel in Alabama Release
Use this form when you are traveling within Alabama either independently or with a group from BSC to represent the College or to earn academic credit. Examples: During the Fall Term, you participate in a field trip to Montgomery as part of your class, or you contract an individualized project to intern in a neuropsychology lab at UAB.
