6. Reduced Schedule/Intermittent Leave
(a)
Reduced schedule or intermittent leaves are subject to College approval unless
medically necessary.
(b) Should a faculty member choose a reduction in course load, the appropriate
reduction of other work, such as committee assignments and advising, will be
determined upon mutual agreement of the faculty member and the Provost of the
College.
7. Advance Notice and Medical Certification
(a)
The faculty member must provide 30-day advance written notice except in
emergency or unforeseeable situations. Failure to give a 30-day advance notice
for a foreseeable leave may result in a 30-day delay in the leave.
(b)
The faculty member must provide medical certification from a health care
provider to support any request for leave to care for the employee's seriously ill
spouse, son, daughter, parent, or due to the faculty member's own serious health
condition. Medical Certification forms will be provided by the College and must
be returned within 15 calendar days.
(c)
The faculty member must provide a Fitness for Duty (Return to Work Statement)
from a health care provider prior to returning to work if the leave was for the
faculty member's own serious health condition.
